Official Contest Rules

1. Eligibility. No purchase necessary to enter. Sweepstakes are open to children adopted through Arizona’s foster care system 6 -18 years old as of May14, 2006 except employees of Gannett Co., KPNX-TV, The Arizona Republic, its affiliated companies (its advertising or promotion agencies), or members of their respective families. Employees and family members of any third party contest participants are not eligible.

2. Entry. Children must submit a one-page essay (maximum of 400 words) describing the theme My Mother, My Teacher. The submission must include the name of the child and a parent, age of the child, date the child was adopted, phone number, mailing address and e-mail address. A photocopy of the Arizona order of adoption must be submitted with the entry. To download a submission form, visit www.lodestarfcc.org. The contest will be divided into two categories – one for children ages 6-10 and another for children ages 11-18 (as of May14, 2006). Submissions can be e-mailed to mothersdayessay@lodestarfcc.org, faxed to 602-212-2564 or mailed to MOTHER’S DAY ESSAY CONTEST at 2320 North 20th Street Phoenix, AZ 85006. Essays must be submitted by 5 p.m. on Friday, April 28, 2006. Gannett Co., KPNX-TV, The Arizona Republic, The Office of the Governor, LFCC and its affiliated companies are not responsible for any and all computer failure or fax/phone line failure.

3. Prizes. The Mothers of the two winning essayists will receive a prize package. Prizes are still being secured at this time. Winning essayists and their Moms will be featured on KPNX Channel 12 and published in The Arizona Republic. Barring spot news. Odds of winning depend on the number of entries received at the time of the contest. Winners will be notified by Friday, May 5, 2006. Winners must be available for interview between Tuesday, May 9, 2006 and Friday, May 12, 2006. In the event the winner cannot complete the interview in this time period, winner will forfeit the prize. Channel 12 reserves the right to award unclaimed prizes at its discretion. Winner must show two (2) forms of identification when claiming prize; one must be a valid photo identification. Winner must claim prize in person. The value of all prizes are non-transferable. There can be no guarantee that the Internal Revenue Service (IRS) or the Arizona Department of Revenue, or its counterpart in another jurisdiction will accept the value of any prizes for purposes of determining any income tax which may be due by the winner. The prizewinner will not have the option of accepting a cash equivalent. There will be no exchange or modifications to the prize.

All decisions by Channel 12, The Arizona Republic, the Office of the Governor, and the LFCC are final. At any time, changes may be made as to the manner of entry or to the prizes awarded.

4. General. Taxes on all prizes are the sole responsibility of winners. No prize transfers. Prize winners must execute an affidavit of eligibility and liability/publicity when picking up prize. Offer subject to federal, state, local regulations, and laws and is void where taxed, restricted or prohibited. By entering, participants agree to these rules and use of winners’ name or likeness for similar promotion without compensation. The Office of the Governor, KPNX-TV, Channel 12, Gannett CO., azcentral.com and all employees are not liable for, and shall be held harmless against all claims against LFCC. The Office of the Governor, KPNX-TV, Channel 12, The Arizona Republic and LFCC reserve the right to modify these rules for clarification purposes without materially affecting the terms and conditions of the contest. The Office of the Governor, KPNX-TV, Channel 12, azcentral.com, The Arizona Republic and LFCC are not responsible for typographical or other errors to the offer or administration of this contest, including but not limited to errors in advertising, the Official Rules, the selection and announcement of the winner or the distribution of the prizes. The Office of the Governor, KPNX-TV, Channel 12, The Arizona Republic and LFCC reserve the right to cancel this contest at any time without obligation or prior notice. By entering, participants release and hold harmless, the Office of the Governor, KPNX-TV, Channel 12, azcentral.com, The Arizona Republic, LFCC their respective parents, subsidiaries, affiliates, directors, officers, employees and agents of any and all liability or any injuries, loss or damage of any kind arising from or in connection with this contest or any prize won. In addition, by entering this contest, entrants accept and agree to be bound by these rules, limitations and/or restrictions, and by the decisions of the Office of the Governor, KPNX-TV, azcentral.com, The Arizona Republic and LFCC.

5. Prize Winners. All participants will receive notification by mail of prize winners by Monday, May 2nd, 2005.

6. Limitations of Liability. By entering the contest, entrant agrees that (1) any and all disputes, claims, and causes of action arising out of or in connection with the contest, or any prizes awarded shall be resolved individually without resort to any form of class action; (2) any claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering the contest, but in no event attorneys’ fees; and (3) under no circumstances will any entrant be permitted to obtain any award for, and entrant hereby waives all rights to claim punitive, incidental or consequential damages and any and all rights to have damages multiplied or otherwise increased and any other damages, other than damages for actual out-of-pocket expenses.

7. Privacy Notice. By entering this contest, each entrant consents to the collection by the Sponsor of the information contained on the entry form, also, all entrants understand and agree that the Sponsor may use the information it collects from entrants to provide those entrants with information on the products and services of the Sponsor. Any entrant not interested in receiving such material, should notify the Sponsor at Channel 12 KPNX-TV, 1101 N. Central Ave. Phoenix, AZ 85004.
